Job Description:
The Program Transition Coordinator will be responsible for overseeing and implementing transition supports and plans of students entering and exiting the InclusiveU program at Syracuse University. The day-to-day management of this role involves communicating with NYS OPWDD, partner agencies, students and families to assist in planning and executing the students next steps when entering or exiting the university; this includes, recommending self-direction budget adjustments, appropriate levels of mentor support, connecting to resources, course advisement, and assistance in planning living arrangements. The Coordinator will be an integral part of the InclusiveU team and be responsible for the direct supervision of a Student Support Assistant.

This role will also be responsible for collaborating with other state agencies and national partners to reduce barriers to student access and provide guidance to families through the admissions process. The Coordinator will also assist in InclusiveU recruitment and marketing by connecting with school districts, community disability agencies, and other educational entities locally and nationwide to help inform others about the program. This role will be responsible for alumni communications and marketing of events to support students, in addition to tracking outcomes to inform research. This role requires strong organizational skills, quick and effective decision-making, creative and independent problem solving, demonstrated initiative, and the ability to communicate effectively with team members and stakeholders. The ideal candidate will be a detail-oriented individual who can manage multiple projects simultaneously and ensure all deadlines are met on time.

Responsibilities:
  • Oversee and implement transition support and plans for students and families by communicating with NYS OPWDD, partner agencies, students and families to assist in planning and executing the students next steps when entering or exiting the university; this includes, recommending self-direction budget adjustments, appropriate levels of mentor support, connecting to resources, course advisement, assistance in planning living arrangements and the direct supervision of a Student Support Assistant.
  • Independently create relationships and lead collaborations with state agencies and national partners to reduce financial and access barriers for students. Provide guidance and support to families through the admission and exit processes to help direct them through next steps. Create and implement a system to track student outcomes post college to inform program milestones, successes and identify areas of improvement.
  • Manage an academic advising caseload to provide individualized guidance to students. Assist students in selecting and registering for courses that align with their academic goals, interests and certificate requirements.
  • Assist in the marketing and recruitment of students to InclusiveU by fostering relationships and providing information to local, regional and national agencies, school districts, and additional educational entities.
  • Facilitate increased engagement of program alumni by expanding access to alumni events, employment opportunities, professional development opportunities, and broader community supports.
  • Additional responsibilities as assigned.
